Calsolaro V and colleagues indicated significant benefits of oral anticoagulants (OAC)
in oldest-old patients with atrial fibrillation (AF) in the previously published article,
entitled “Long-term effectiveness and safety of anticoagulation therapy in oldest
old, frail people with atrial fibrillation [

].” Since AF is the most common sustained cardiac arrhythmia related with worse clinical
outcomes [
[2]
] and is increasing in prevalence and incidence worldwide in this ageing society [

], clarifying the best care for these patients is crucial. Furthermore, older patients
tend to have higher risks regarding both embolism and bleeding, their investigation
