      A 49-year-old woman of unremarkable past health was found comatose in her car. Blood pressure was 130/70 mmHg, pulse 80 beats per minute. There was no circumstantial evidence of drug overdose. Urine toxicology tested negative. Electrocardiography (ECG) showed anterolateral ST-segment elevation (Fig. A). Head computed tomography (CT) of demonstrated no intracranial pathology. Non-sustained ventricular tachycardia developed shortly. Echocardiography revealed left ventricular dysfunction with akinesia of mid-ventricular walls but sparing basal and apical segments (Fig. B, video).

        An 86-year-old woman is evaluated for an abnormal electrocardiogram (ECG) during admission for hemoptysis (Figure 1-A). She denied any chest pain. Other medical problems include bronchiectasis, paroxysmal atrial fibrillation and sinus pause for which a dual-chamber pacemaker was implanted four years prior. Coronary angiogram performed two years prior for atypical chest pain showed mild disease in proximal left anterior descending artery. ECG one year prior is shown (Figure 1-B). What is the diagnosis?

          A 76-year-old man presented with a painful rash one week after receiving chemotherapy consisting of cetuximab, oxaliplatin and fluorouracil for metastatic colon cancer. Physical exam was notable for pustules on his neck, chin and upper chest (Fig. 1). He was afebrile.
